Neil Patrick Harris definitely had scheduling issues. It has been said that Ken Jeong had ethical concerns, namely he doesn't just play a doctor on tv, and quite frankly, I doubt Bill Cosby was even asked at all.
Dick Van Dyke and Robert Picardo would have been cool additions as well.
I believe they were trying to limit casting to one doctor per series, so House was pretty well covered. There are way too many tv doctors to represent every series, but I guess a sequel isn't out of the question.
I like this one, too.
